We provide this on-line Seller's Preliminary Septic Information form as a fast and easy way for Sellers to submit preliminary information about a property to be inspected - this information is required before we can physically inspect the septic system. The information form can be submitted by either the seller, the listing agent or other authorized seller's representative.

Answers are required in all fields. If you do not know the answer to a question, write "Don't Know" or "Unknown." After you have completed the form and answered the "Human Test" question, just click the "Submit" button at the bottom of the page and the information will be instantly transmitted to our office; there's no need for you to fax or mail anything.

When the completed form has been successfully submitted to our office, you will see a green "Success" message.
